Student Eligibility Reports

Policies and Procedures for filing student eligibility report have been revised beginning with the 2009-2010 school year reports. Please be aware of these changes:
- Only reports submitted on new forms (Revised 4/09) will be accepted. This applies to both Form A and Form B. Please discard your outdated certificates of eligibility.
- Students must pass 2.5 Carnegie units per semester to maintain eligibility. Notations in the "Units Accumulated Previous Semester" column must be reported in this manner; not in courses passed.
- Second year students (those entering in Fall, 2008) must have 5 Carnegie units total to be eligible.
